How to Get Started
For routine appointments, please call your local CVHS office location.
If you are an established patient and need urgent medical care after hours, please call your local office number and follow the instructions for reaching the after hours on call physician. For medical emergencies, go the nearest hospital emergency room or call 911 immediately!
Patient Registration Information
Please arrive at least 30 minutes early to your appointment to fill out paperwork. Or, download and complete form below and bring with you to your appointment.
Please be sure to bring with you the following items:
- Insurance card
This includes private insurance, Medicare, Medicaid, or any other insurance card.
- Proof of income (if applying for sliding scale)
Please bring income for all household members. We will accept your last three pay stubs, a copy of last year’s tax return, or a letter from your employer verifying how much you make. If you receive benefits such as unemployment, social security, retirement, child support, or disability please be sure to bring in your award letter.
- Current medications
Please bring all of your current medications.
Sliding Fee Information
Central Virginia Health Services is able to discount many dental, medical and pharmacy charges based on a family’s ability to pay (income). In order for a family or individual to be considered for this discount, certain income information must be provided (see above for a list of approved items). This information must be provided within 30 days of application, or the full fee will be charged. The amount of the charge not discounted must be paid within 30 days of the clinic visit. The Patient Services Representative is available to assist you if you would like to apply for the sliding fee.
The guarantor is expected to pay CVHS’s cashier for all medical/dental services, drugs or other services received from CVHS before leaving the Clinic. If a hardship prevents immediate payment, all payments are due within 30 days. Your insurance company will be billed and if there is an overpayment, you will receive a refund. Accounts not paid in full within 30 days will be considered overdue, and CVHS will take action to collect all fees. Any guarantor who feels that he or she will have difficulty paying the charges should make payment arrangement prior to seeing the dentist or doctor.
CVHS sites accept all major insurances, including: Medicaid, Medicare, Medicare Advantage, and Tri Care. We also accept most major dental plans. For more information, please contact a Patient Services Representative at your site.
We accept cash, check, money order, MasterCard, and Visa. You may pay at the site or pay over the phone at our Corporate Office. To pay over the phone, please call 434-581-3271, ext 10721.
The Billing Office is open Monday through Friday from 8:00 to 4:30. You can reach the business office by calling 434-581-3271.
